Free parking is available in the surface lot. Note: Patients under 18 years of age must be accompanied by a parent or guardian.
Consultation Checklist
Please bring your surgical referral slip, a list of current medications, and completed insurance forms. Alert us of any heart conditions or diabetes prior to surgery.
X-Ray Coordination
Have your dentist forward recent x-rays, or bring them with you. If additional diagnostic films are necessary, they can be taken directly at our facility.
